Healthcare in Portugal
Who is covered, from what date, and what insurance you need
Legal residents register with the national health service and pay little or nothing at the point of use. Waiting times for non-urgent specialist care are long enough that most foreign residents also carry inexpensive private insurance.

Getting registered
Take your residence permit, tax number, social security number, and proof of address to your local health centre and request a user number. Once issued you are assigned a family doctor, subject to availability, which in some districts means a wait.
Before your residence permit is issued you are not in the system, which is why every visa category requires private travel or health insurance for the initial period.
Why residents buy private cover anyway
Public emergency and hospital care is competent and effectively free. Non-urgent specialist appointments and elective surgery can involve long waits.
Private insurance in Portugal is inexpensive by American or British standards, and private consultations without insurance are commonly in the range of fifty to a hundred euros. The usual arrangement is public cover for serious events and private cover for convenience.
Pharmacies and medication
Pharmacies are widespread and pharmacists dispense a wide range of medication with prescriptions issued electronically. Many prescription drugs are state-subsidised, and prices for common medicines are markedly lower than in the United States.
